Buttock augmentation with fatty tissue (Brazilian Butt Lift or BBL)


Buttock augmentation with fatty tissue has become one of the cosmetic surgeries most demanded by patients. It allows to improve both the shape and the size of the buttocks through the use of infiltrations with the patient’s own fatty tissue. The fatty tissue is obtained through liposculpture (put link of liposculpture), so the effect is double, since on the one hand the patient obtains an improvement in her silhouette by removing the fatty deposits, and on the other by contrasting with some buttocks more shaped, bulky and pleasant.


The surgery begins with a liposculpture, in which all or part of the fatty tissue of the patient will be obtained, the amount obtained will depend on the result that the patient wishes. If the patient wants very large buttocks and has a sufficient amount of fatty tissue in the body, sometimes it is not possible to put the entire amount in a single surgery, so she would need two interventions, allowing to pass between 4 to 6 months between them. Furthermore, with liposculpture, the patient’s musculature can be defined if she is a candidate for said surgery. After liposuction is completed, the fatty tissue is carefully processed and very carefully re-injected into the buttock area. The injection is the most delicate part of the procedure, since both the result and the absence of complications depend on the careful technique and art of the surgeon.

This surgery involves some significant risks, so intramuscular fat injection is currently completely contraindicated. Hence the importance of performing it by a surgeon experienced in the procedure, with good instruments, under sterile conditions in the operating room and in a hospital that has all the safety protocols.


Not all patients are suitable for this surgery. They must have a BMI less than 30, being the most overweight patient in whom the result may be more voluminous given that there is a greater amount of fatty tissue. Likewise, in thin patients with some reserve of fatty tissue, surgery can also be performed, seeking in this case to improve the shape, rather than give volume. On the other hand, it is essential to have slightly flaccid skin, as well as to have healthy lifestyle habits, since after surgery it is very important to maintain a stable weight and exercise, in order to optimize the results of the intervention. If the patient has a lot of sagging skin, this type of surgery is not usually recommended unless the patient understands the limitations of the procedure.


This type of intervention must be performed in a hospital that meets all health requirements and has an intensive care unit. Cosmetic surgery is a very regulated surgery with fewer complications, but in case they appear, it is essential to have the support of a good hospital. Hence, low-cost surgeries try to save money on this and are performed in small clinics where these safety conditions are not covered.

The realization of this type of intervention must be performed by a plastic surgeon with experience in body contouring surgery. On the other hand, buttock augmentation with fat is a surgery that can entail vital risks for the patient if it is not done carefully and with a good understanding of the anatomy, so great care must be taken when performing it, taking a good preparation in this procedure.


After the intervention, it is usual and completely normal for the patient to feel a sensation of stiffness in all the intervened areas, this usually normalizes after 3 days. On the other hand, it is common for inflammatory fluid to leak through the small holes used to extract the fat, this can last several days and is completely normal and desirable.

The patient must simply keep the small wounds clean and take the prescribed medication at the time of discharge from the hospital.

You will not be able to sit or sleep on your back for the first few weeks. This is of vital importance to achieve a good result, given that due to the pressure, the fat tissue could be mobilized and not receive a correct blood supply, so that part of the grafted fat could be lost and the desired result not obtained.


Liposculpture is not a surgery in which there are large detachments or scars, so your recovery is faster than other cosmetic surgeries. However, despite having a reputation as a “less aggressive” surgery, it can carry risks that can be serious if the surgeon does not have the proper training and experience. In order to avoid them, the surgeon must be highly qualified, operate in approved centers and use the appropriate and properly sterilized material and instruments.

Another type of complication, slight but more common, is usually asymmetries or irregularities in the skin, caused by the poor application of the technique, which has been carried out in a very superficial way, in a very traumatic way or in an inappropriate way. This causes the skin to be flaccid and with ripples, which are often very difficult to correct. It does not reverse severity on the patient’s health, but it does affect the aesthetic result of the surgery in a very negative way.


The patient should undergo a lymphatic drainage massage regimen during the first weeks, as well as using the appliances to help drain edema, that also improves skin retraction.

The patient can return to work after seven to ten days using compression garments under their clothing.
